THE ASSESSMENT OF CYTOKINE PATTERN IN PATIENTS WITH SEVERE SEPSIS USING THE BIO-PLEX-PROTEIN ASSAY SYSTEM

The goal of current study was the multiplex quantitative measurement of 18 cytokines in sera of patients with severe sepsis, including septic shock. The development of combined T-cell/monocyte immune disorders due to the cytokine balance shift following the increase of immunosupressive and anti-inflammatory mediators was revealed in patients (n = 14) with severe sepsis. The measurement of cytokines using ELISA and Bio-Plex assay showed that in contrast to healthy donors the septic patients were characterized by increased serum level of IL-1Ra, total IL-10, IL-6 and MIP-1b, whereas the concentrations of pro-inflammatory cytokines (IL-1b, TNFb, IL-12, IL-17) were near-normal. The elevated levels of IL-8 (50 % of cases), MCP-1 (28,6 %), GM-CSF (35,7 %), and G-CSF (21,4 %) were detected in some patients. The high concentrations of IL-1Ra, total IL-10 and MIP-1b were associated with severity indexes including Acute Physiology And Chronic Health Evaluation (APACHE-II) and Sepsis-related Organ Failure Assessment (SOFA) scores. It was also shown that the septic shock patients were characterized by 1) more prominent depression of T-cell proliferative response in vitro, 2) marked decrease of HLA-DR expression in monocytes, 3) enhanced supressive and anti-inflammatory activities of serum in a bioassay, and 4) higher level of IL-1Ra, total IL-10, IL-6, IL-8, G-CSF, GM-CSF, MCP-1 and MIP-1b. Our data indicate the predominance of anti-inflammatory/immunosupressive reactions in patients with severe sepsis that are "masked" by the clinical signs of systemic inflammatory response syndrome (SIRS).
