Prognostic value of immunoglobulin and cytokine levels of oral fluid in patients with chronic destructive processes of jaws

The aim of the study was to identify the associations between the time course of oral liquid IL-4, IL-8 and immunoglobulins levels and the effectiveness of surgical intervention in chronic destructive processes in the jaw. 18 patients (12 with chronic destructive osteomyelitis and 6 with bisphosphonate osteonecrosis) were subjected to surgical intervention which included sequesterectomy, osteosynthesis, and guided tissue regeneration. Before and after surgery, sIgA, IgG, IgM, IL-4 and IL-8 concentrations were measured in the oral fluid. It is shown that chronic destructive processes of the jaw are accompanied by a significant increase in IL-8 and IL-4 in the oral fluid. If successful, the treatment reduced initially high levels of IgG, slgA and IL-4, while IL-8 levels remained high. The imbalance of pro- and anti-inflammatory cytokines, while maintaining high levels of immunoglobulins in oral fluid, may be a sign of the ineffectiveness of the treatment and the possibility of future recurrence of osteonecrosis.
